BHS Registration Requirements

Each player will need to complete the steps listed below to participate with the BHS team:


1. WIAA Sports Physical Form Completed By a Physician:

A physical exam must be current within the past 24 months and this form needs to be filled out and signed by a physician. If the player's physical expires during the high school season, they will not be able to participate until a new physical exam form is received by the coach. Completed physical forms should be emailed to our Registrar. WIAA's physical form can be printed from this link.

2. Medical Warnings:

Please download and read the following pamplets. You will need to ackbnowledge that you read them during the registration process:
a. Concussion Pamplet
b. Sudden Cardiac Arrest Form

3. USA Water Polo Membership:

All players must have a USA Water Polo membership to participate. Your membership provides the club with the required insurance in the event of an accident. Go to www.usawaterpolo.org to purchase your membership. The Bainbridge Water Polo Club # is 267. A "bronze athlete" membership is sufficient for the club's requirement for local games. If you are on a traveling team, you can upgrade to a "silver athlete" membership at a later date.

4. Online Registration:

Parents must complete the online registration process and pay fees for either High School Boys or High School Girls.

5. WIAA Athletic Elegibility:

To meet the WIAA Athletic Eligibility during the High School Season, the players will be responsible for completing a progress report form (given to them by their coaches) to confirm their athletic eligibility for the Season. The players will need to receive progress update and signatures from their teachers and submit the completed form to their coach. The coaches will notify players when the forms are due to be eligible for Regional/State tournaments.

6. Inherent Risk Acknowledgement:

Please download, read and sign the following form. Email completed form to our Registrar.

6. Athlete Information Form

If we do not have an Athlete Information Form on file for you, please fill out and sign. Email completed form to our Registrar.

Due to budget constraints with the Bainbridge High School (BHS) Athletic Department, the high school has decided to have ALL club sports** removed from their responsibility. BHS will no longer be a conduit for the player's physical forms, volunteers/driver's background checks or checking player's academic grades. The High School players will only register with Bainbridge Water Polo Club. Bainbridge High School's Policy concerning Club sports can be viewed at the BHS athletic website.

Parent Volunteer Drivers

We rely on parents to drive our players to games, and we need your help. Please look for an email from the Driving Coordinator and help sign up for some time.

Due to BHS requirements, drivers must complete the volunteer driver form and bring copies of their driver's license and insurance.
